INSURANCE COVERAGE
We are health care providers for several insurance companies. We require payment at time of service for all new patients and
for out of area patients. We will provide you with an insurance claim form which you can use for reimbursement.
INSURANCE VERIFICATION
For our established patients, we will verify your insurance coverage and bill you. Your deductible, co-pays and uncovered services are your responsibility.
MEDICARE
We accept assignment on all Medicare patients. We do not bill secondary insurance, therefore your deductible, co-pays and payment for uncovered services need to be paid at time of
service. If we receive direct payment from a secondary insurance company we will send you a refund.
WORKERS COMPENSATION INJURIES AND EMPLOYMENT PHYSICALS
We verify coverage prior to seeing each patient and bill the appropriate parties. We have been working with employers for over twenty years and are well versed in
the filling of Doctors’ First Reports, Work Status Reports and any other information as needed.
PAYMENT
We accept cash, checks, ATM, Traveler’s checks and all major credit cards at the time of service.
We are sorry but we are not authorized to accept Medi-Cal patients.
